Case Manager
We are looking for a Case Manager Join us in a part-time capacity to meet our growing business needs.
Position Purpose:
The main role of the Care Services Manager (CSM) is to foster positive connections with potential clients from the start and continue to build and nurture relationships with our clients and their families across their time with us. You will work with clients to design appropriate Care Plans to meet their needs, provide them with prompt contact and ongoing reviews to ensure that their care needs and goals are met and that services are adjusted as needed.
The CSM ideally possess a good understanding of Home Care Packages and NDIS. This may include knowledge of a range of other community services available that can assist clients and their families.
Job responsibilities:
- End to end client services management: intake, service (and risk) assessment, reviews, service transfer and coordination of direct support activities and understanding customer’s monthly statement.
- Attend Consumer's home for assesments and collobarating on planning.
- Working with Consumers, their supports and other providers to achieve their goals in line with their individual plans.
- Maintain accurate and up to date case notes on customer progress.
- Maintain an up-to-date knowledge of local services and pathways that customers can access and current sector systems and practices.
- Facilitate care worker support and supervision including accurate documentation, record keeping.
- Maintain internal filing, including accurate client documentation and other miscellaneous filing.
- Complying with all relevant legislation
- Preferbly Female.
- Relevant tertiary or vocational qualifications in community and social service management or equivalent.
- Minimum 1 years’ experience in similar role.
- Demonstrated leadership skills and experience working with case management workforce (clinical and non-clinical) within a community setting, managing complex client care scenarios.
- Excellent interpersonal, influencing, negotiation and facilitation skills and proven ability to successfully engage a diverse range of stakeholders.
- Commitment to managing service delivery within a client focused model of care.
- Well-developed IT skills and experience with digital solutions.
- Driver’s license and ability to travel.
- Nursing background & qualifications (Preferred)
