Pay: $80,000.00 – $95,000.00 per year

Job Description: We are seeking a highly skilled and compassionate Support Coordinator/Case Manager to join our team. The ideal candidate will be responsible for helping participants navigate their NDIS plans, coordinating supports, and ensuring that they receive high-quality services.You will work closely with clients to assess their needs, develop support plans, and connect them with the necessary resources and services.
Key Responsibilities:
- Assist clients in understanding and implementing their NDIS plans.
- Coordinate a range of support services, including personal care, therapeutic supports, and community participation activities.
- Develop, review, and update individualized support plans based on clients' needs and goals.
- Collaborate with other healthcare and social service professionals to ensure comprehensive care.
- Advocate for clients to ensure they receive appropriate and timely support.
- Provide ongoing support and guidance to clients, helping them overcome barriers and achieve their goals.
- Maintain accurate and up-to-date client records and documentation.
- Monitor and report on the progress and outcomes of support plans.
Essential Requirements:
- Diploma level qualification in a relevant field (e.g., Social Work, Disability Services, Community Services).
- Minimum of 2-3 years' experience in the disability sector or a related field.
- Strong understanding of the NDIS and its processes.
- Excellent verbal and written communication skills.
- Demonstrated ability to work independently and as part of a team.
- Compassionate, empathetic, and dedicated to making a positive difference in the lives of individuals with disabilities.
- Clear police check.
- Fully vaccinated against COVID-19.
Preferred Qualifications:
- Experience in case management or support coordination within the NDIS framework.
- Proficiency in a second language.
