About the role

Permanent full time opportunity to provide social work clinical leadership within STARS, and requires the independent application of clinical expertise in a highly complex and varied patient population. The role is central to ensuring quality discharges, maintaining patient flow within the geriatric and rehabilitation service lines within STARS that includes patient cohorts with a prolonged length of stay relating to a range of complicating factors, primarily NDIS, transition to RACF with complicating factors and convalescing patients; inclusive of supporting colleagues and the provision of within scope clinical advice.

About you

Our ideal candidate will be someone who can carry out the following key accountabilities in accordance with the Metro North Health's values:

  • Integrate advanced level of knowledge, evidence-based skills and reflective practice in the delivery of clinical services to enable the NDIS pathway, independently applying professional judgement to problems of significant scope within an increasingly complex and critical service.
  • Deliver advanced level clinical services across multiple specialities with minimal clinical supervision.
  • Apply valid, reliable evidence and high level professional judgement to problems of significant scope within a complex caseload and maximise opportunities to reduce barriers to discharge
  • Lead quality improvement initiatives for Social Work within STARS, and actively contribute to the broader quality initiatives within STARS, MNHHS and/ or state-wide initiatives that may be within scope
