An exciting opportunity is available for a Clinical Nurse Consultant to join the Primary Care and Community Health department. This position will work in close liaison with staff from Primary Care and Community Heath, acute facilities, General Practitioners, Specialists, other healthcare providers, Non-Government agencies and consumers and their carers and families to improve outcomes for people with a Complex Aged Care condition.
