Community Services Coordinator

• Care/client coordination including care plans and support to upper management.
• Responsible for rostering and supervising community support workers.
• Intake management and document completion
• Support in the areas of policy, procedure and compliance
• Facilitation of client community inclusion and participation
• Promotion of client independence and choice and control
• Overseeing implementation, management and coordination of Work Health Safety and Return to Work Matters
• Developing and implementing educational, training and support programs
• Overseeing of Work Health Safety and Return to Work matters
• Collaborating with community and government organisations
• Preparation of funding submissions, resource applications and reports for governing bodies and agencies
• Adherence to industry legislation and standards

• Possess and demonstrate sound experience, knowledge and skills in disability and aged care
• Diploma of Community Services with at least 2-3 years of industry experience
• 2-3 years of relevant supervisory experience
• Experience in a Work Health Safety context
• Valid Victorian Working With Children's Check
• Valid Victorian Police Check
• Current First and CPR competency
• Valid Victorian Driver's license, registered and roadworthy vehicle, full comprehensive insurance.

**Not required but strongly favourable to have an Advanced Diploma in Community Sector Management
