About the Role
In this full-time role, you will be responsible for providing high quality care and support to our participants in their homes and in the community in Hamilton.
Key ResponsibilitiesProvide direct care assistance to each individual in accordance with their service plan.
Provide support to participants to maximise their participation in age appropriate activities and tasks including in-home support and community access.
Support participants to access activities and outings in the community.
Provide direct care assistance to each participant as detailed in their service plan and as directed by the Manager.
Monitor the safety and well-being of participants and report any concerns to the Manager.
Provide 1:1 or small group training and support in response to individual participant needs.
Ensure completion of appropriate documentation as and when required.
Ensure administration of medications to participants in accordance with the Medication Management Policy and Procedure.
Display a commitment to the Mission, Vision and Values of Prohealth Community Care Services.
Mandatory RequirementsRelevant qualifications in Disability Support or related fields
At least one year experience assisting with ADL's
Manual handling and medication administration experience
Working with Children Check (or able to obtain one)
First Aid certification
Your own reliable vehicle and valid Driver's licence
Flexibility to work at multiple locations and sites.
