Assertive Outreach Case Manager
About the role
This role as a Darebin Assertive Outreach Case Manager is all about connecting with and supporting individuals experiencing homelessness, particularly those sleeping rough or at risk. Through outreach in the Darebin community, you’ll identify people in need, link them to essential services, and provide tailored support to help stabilise their lives.The role’s purpose is to bridge the gap for those facing homelessness by offering meaningful, sustained assistance, including housing referrals, access to health services, and other essential resources. Your work will directly impact lives, helping people move toward greater stability and hope, while also contributing to MOSS’s mission of ending homelessness.
We’re looking for a candidate with a qualification in community services, social work, or a related field, along with experience in assertive outreach or homelessness support. You should be collaborative, culturally sensitive, and committed to social justice, with strong advocacy and case management skills.
