A Community Lead Worker at Relieve Disability Service designs and delivers capacity-building programs that connect Relieve participants with their communities. The role conducts holistic needs assessments, drafts individual participation plans, and coordinates multidisciplinary supports across nursing, therapy and daily living services.

It leads outreach events, supervises disability support staff, and monitors service quality through reviews and client feedback. By combining frontline leadership with advocacy, the position turns NDIS funding into measurable social and personal outcomes.

Key responsibilities
  • Conduct strengths-based intake and needs assessments with participants and their families, capturing goals around community inclusion, daily-living skills and capacity building.
  • Design, document and review individual Community Participation Plans that translate assessed needs into weekly activity schedules, funding utilisation forecasts and measurable outcomes.
  • Coordinate multidisciplinary professionals (e.g. physiotherapists, social workers, nurses) to deliver holistic care across the company’s service lines.
  • Lead group workshops and outreach events including social-skills clubs, travel-training, gardening programs and cultural-language meet-ups to foster inclusion and raise community awareness of disability supports.
  • Supervise and mentor junior disability and community support workers
  • Collect, analyse and act on participant feedback and service-outcome data to drive continuous improvement, update risk registers and meet NDIS Quality & Safeguards Commission reporting duties.
  • Undertake community-risk and safeguarding assessments for short-term accommodation sites, group activities and in-home supports; implement WHS and incident-response protocols.
  • Lead evidence-based advocacy campaigns (newsletters, local forums, social-media outreach) to promote inclusive practices and maximise take-up of Relieve’s NDIS programs across the Wyndham region.
Qualifications
  • Bachelor’s degree or higher qualification in a related field.
  • Minimum 1 year of relevant experience
  • Ability to work independently and as part of a team, demonstrating excellent communication and interpersonal skills
  • Attention to details and precision in all aspects of work
