As our services continue to expand, we are seeking a dedicated and compassionate Community Worker to join our team based in Tarneit, VIC.
The Opportunity
This role is ideal for a Community Worker with experience in NDIS support, aged care services, or community-based social work. You will work closely with clients to provide tailored assistance, promoting independence, inclusion, and quality of life.

Key Responsibilities- Deliver personal care, domestic assistance, community access support, and companionship to NDIS participants and aged care clients.
- Facilitate client participation in social, educational, and recreational activities, enhancing community integration.
- Implement and support individual client care plans, ensuring goals are met with flexibility and empathy.
- Advocate for the rights, choices, and dignity of all clients, promoting self-determination.
- Adhere to NDIS Quality Standards, Aged Care Standards, and occupational health and safety regulations.
- Maintain accurate case notes, service records, and incident reports according to organizational policies.
- Liaise with care coordinators, healthcare providers, family members, and external stakeholders to ensure holistic support.
Qualifications & Skills
- Diploma in Community Services, Individual Support (Disability or Aged Care), or a related field (highly regarded).
- Minimum 1–2 years of experience supporting NDIS participants, aged care clients, or delivering social services.
- Understanding of NDIS Practice Standards, Aged Care Quality Standards, and client-centered care principles.
- Strong interpersonal and communication skills with a genuine passion for community empowerment.
- Ability to work autonomously and flexibly in line with client needs and service requirements.
- Current driver’s license and access to a reliable vehicle.
- Current First Aid and CPR certificates (or willingness to obtain).
- Valid NDIS Worker Screening Check and Working with Children Check (or willingness to obtain).
- Full working rights in Australia are required.
