Hope and Care Pty Ltd is a multi service provider in home care for both elderly and/or those with a permanent disability, established to meet the needs of participants in South Western Sydney. We provide high-quality, person-centred support services to eligible consumers.

Full Time Aged / Disability Carer
$40 – $50 per hour on casual basis

$75,000 - $100,000 – casual basis.

RESPONSIBILITIES AND DUTIES
  • Ensure robust internal and external stakeholder management at a service level and be recognised as the Subject Matter Expert.
  • Deliver high-quality services that support clients' dignity and opportunities to be accommodated in a caring, compassionate, and secure manner.
  • Maintain a client focus through the delivery of services using the person-centred active support model and assist clients in accessing mainstream services.
  • Promote the dignity and positive image of clients within their communities, maximising their opportunities to participate in and contribute to the communities in which they live.
  • Attend multi-disciplinary service meetings, case conferences, and reviews, and undertake case coordination and reporting as necessary.
  • Identify the need for behavioural support plans and implement them to ensure individual changing needs are met.
  • Monitor the correct administration of prescribed medication in accordance with organisational guidelines.
  • Collaborate with the rostering team to develop and implement rosters centred on the needs of the client, including troubleshooting timesheets, managing leave requests, and filling and publishing rosters in advance.
  • Manage daily rostering, including filling shifts and making day-to-day changes to ensure service continuity and client support.
  • Ensure effective matching of clients with support workers to support the achievement of client outcomes.
  • Participate in rotational "On-Call" duties to ensure effective service delivery outside of normal business hours.
  • Support the Supports Manager with investigative processes and disciplinary actions.
  • Lead service meetings, ensuring they are coordinated, chaired, minuted, and actions are followed up and completed.
  • Maintain documentation relating to clients, adhering to the highest level of confidentiality following the person-centred active support framework.
  • Provide input into the development of organisational best practice standards, policies, procedures, and continuous improvement initiatives.
  • Provide immediate and first-line critical incident support.
  • Comply with all legislative and organisational policies and procedures, ensuring awareness and compliance within the team.
  • Provide timely reports to inform continuous improvement strategies at a service level.
  • Apply WHS legislation and organisational requirements to create and manage a safe work environment.
  • Work collaboratively with peers and colleagues across the organisation to achieve organisational objectives.
REQUIRED SKILLS
  • Minimum of three (3) years' experience in a disability setting.
  • Prior experience in a leadership or supervisory role.
  • Comprehensive knowledge and understanding of the National Disability Insurance Scheme (NDIS) and NDIS Quality and Safeguards Framework.
  • Relevant tertiary qualifications in disability, community services, or a related field.
  • Current First Aid Certificate.
  • Working with Children Check (WWCC) clearance within the last five (5) years.
  • NDIS Clearance Check
  • Valid driver's licence and a suitable vehicle.
  • Right to work within Australia.
  • Completion of the NDIS worker orientation module.
