Job description:
  • Assess individual client needs and develop personalised care plans.
  • Implement educational, training, and support programs tailored to each client's requirements.
  • Assist clients with daily activities, including personal care, meal preparation, and medication management.
  • Encourage and support clients in developing life skills to promote independence.
  • Monitor client progress and adjust care plans as necessary.
  • Maintain accurate records and report on clients' well-being and any incidents.
  • Facilitate clients' participation in community activities and social events.
  • Support clients in accessing community resources and services.
  • Work closely with families, healthcare professionals, and other stakeholders to ensure comprehensive care.
  • Participate in team meetings and contribute to continuous improvement initiatives.
  • Adhere to the National Disability Insurance Scheme (NDIS) guidelines and Eden Care's policies and procedures.
  • Ensure all practices comply with relevant health and safety regulations.
Qualifications/Attributes:
  • Must hold a Diploma of Residential Care or a related field. or higher, or at least 3 years of relevant work experience in lieu of the qualification.
  • Must possess at least a minimum of 1 year of relevant work experience.
  • Strong interpersonal and communication skills.
  • Ability to work collaboratively in a team environment.
  • Empathy, patience, and a genuine commitment to supporting individuals with disabilities.

Salary Range: $70,000 - $85,000 p.a. excluding superannuation.
