South West Care Services Pty Ltd is a registered NDIS Service Provider. We specialise in providing In-Home Care and Community Access to our Aged and Disability participants throughout Central Sydney and Greater Sydney.
As a Support Worker you will work with empathy to complete a variety of domestic, social, respite and personal care services to ensure a quality life with dignity, respect, and social inclusion.

Responsibilities include:
- Support participants in a social, recreational outings and domestic duties.
- Transporting and accompanying participants to access community activities and services.
- Delivering personalised active support in a one-on-one setting.
- Following daily tasks instructions based on the participants needs.
- Providing personal care, food preparation and domestic cleaning.
- Comprehensive knowledge of the NDIS support items under the NDIS Guidelines.
- Direct involvement, assessment and support of clients ensuring Fellowship policies and procedures are adhered to.
- Develop and maintain positive working relationships and linkages with community agencies, clinical services and all other relevant providers to ensure effective communication, referrals and care coordination can occur.
- Working one-on-one or in a group setting supporting clients to make their own decisions, overcome barriers and strive towards achieving their goals
- Advocate on behalf of clients to access services within the community.
- Monitoring and reporting including monitoring participant referrals, progress against NDIS Goal Plans, data collection and reporting where required.
- Mandatory administration tasks including data entry into the client management system.
- Undertake ongoing professional development as required.
Role Requirement:
- Qualification in Individual Support, Disability, Community Services or Aged Care.
- Current NSW driver’s license
- Current First Aid Certificate
- Current Working with Children Check (or willing to obtain)
- Police Check (or willing to obtain)
- NDIS Workers Check
- Registered Vehicle (not a must)
- COVID-19 Vaccine Digital Certificate
