Position Overview:
We are currently seeking a dedicated and experienced Disability Care Officer to join our growing team. As a Disability Care Officer at VPC Care, you will play a crucial role in coordinating and delivering high-quality support services to individuals with disabilities.
Your responsibilities will include developing care plans, liaising with clients, their families, and other healthcare professionals to deliver tailored care plans that promote independence, inclusion, and overall well-being.
What you'll be doing- Develop, implement, and review individualized support plans tailored to each participant’s unique goals, preferences, and needs.
- Assisting NDIS participant with daily living activities such as bathing, dressing, medication management and meal preparation
- Monitoring and documenting residents' health, behaviour and progress, and reporting any concerns to the nursing team
- Organising and facilitating social, recreational and therapeutic activities to enhance participants’ wellbeing
- Maintaining a clean, safe and comfortable living environment for all residents
- Communicating effectively with residents, their families and the wider care team
- Adhering to all relevant policies, procedures and regulatory requirements
- Relevant qualifications or experience in related field
- Experience and knowledge in a disability care or similar setting
- Excellent communication, interpersonal, and organizational skills.
- Strong problem-solving abilities and the capacity to make sound decisions.
- High level of empathy, resilience, emotional intelligence, and cultural sensitivity
- Strong organisational and time management abilities, with the capacity to multitask effectively
