The Client

Convenient Support Services is a nursing and disability support company that was started in response to a growing need for an organisation that is passionate about services that truly reflect person-centred practice

They are now looking for a Care Coordinator to join their esteemed team.

The Role

As a Care Coordinator at Convenient Support Services, you will be instrumental in designing and implementing care plans tailored to the unique needs of our clients. Your expertise will ensure that all services are delivered in compliance with NDIS guidelines and are aligned with our clients' goals and preferences.

You will work alongside other Care Coordinators, and a specific number of team leaders will report to you.

This is a full-time role with an annual salary set at $100,000 inclusive superannuation. The role is based in West Perth, WA.

Responsibilities

Assess client needs and collaborate with clients, families, and healthcare professionals to develop comprehensive care plans.

Ensure the implementation of these plans, adjusting as necessary to meet client needs and preferences.

Act as the main point of contact for clients and their families, ensuring they are informed and supported throughout their interaction without services.

Address any concerns and ensure client and family satisfaction.

Coordinate with service providers and internal teams to ensure seamless service delivery.

Schedule and monitor in-home visits, therapy sessions, and other support activities according to the care plan.

Monitor service delivery to ensure compliance with NDIS guidelines and our own standards of care.

Participate in regular audits and contribute to quality improvement initiatives.

Work closely with healthcare professionals, service providers, and internal teams to promote an integrated approach to client care.

Share insights and feedback to enhance service effectiveness.

Maintain detailed records of care plans, client progress, and service delivery outcomes.

Prepare reports for internal use and compliance purposes.

Advocate for clients’ needs within the organisation and interactions with external providers.

Ensure that clients’ rights and preferences are always respected and prioritised.

Requirements

To be successful in this role, you must have:

A minimum of a Diploma of Nursing or equivalent

A minimum of 6 year of relevant experience gained in a commercial setting

At least 6 years of experience and knowledge of disability support

At least 6 years of experience working in the area of complex disability support with people

Valid First Aid and CPR certification

Current National Police Check (within 3 years)

Working with Children Check

NDIS Worker Screening clearance

Valid Driver's License

Completion of NDIS Worker Orientation Module

Meeting physical fitness requirements for lifting and transferring clients.

Dependability and punctuality.

Effective communication skills with clients, families, and healthcare professionals.

Cultural sensitivity and respect for diverse backgrounds.

Documentation skills for keeping client care records.

Be able to handle pressure in a professional manner

Have positive past employment work references
