Disability Care with minimum 1 YEAR of exp. We are looking for a compassionate, reliable & exp
Support daily living, promoting cooking, housekeeping, hygiene, shopping, Medication administration, Support participants in building their confidence.
Liaise and collaborate with clients, their community and community & disability support agencies.
Monitor and review the participant's progress through the service and against the support plan.
Utilise positive behaviour strategies
Support clients to participate in age-appropriate activities and tasks, including employment, in-Work in
accordance with the medication administration policy, work health and safety procedures Monitor the
safety and well-being of people with disability and report any concerns to the Manager.
Ensure completion of appropriate documentation. As required, relevant information on disability
Participate in the Lifestyle Service Planning process for individual clients and monitor outcomes.
Work with clients, family/ carers, relevant case managers and other professionals to support the Liaise
extensively with other staff to ensure the overall smooth and consistent running of the Report regularly to
delivering quality and safe support services to people living with a disability, their families, and carers.

We are currently looking for qualified disability support worker/Personal Assistant/ AgedDisability Care with minimum 1 YEAR of exp. We are looking for a compassionate, reliable & exp
disability support workers for our clients.
Annual Salary offering - $73,150- $82,000Full-time positions
Key Responsibilities:
