Right at Home Sydney Central and Eastern Suburbs provides high-quality in-home care and assistance to our valued clients in the Sydney Central and Eastern Suburbs. We support people living with Alzheimer’s, dementia and cognitive decline, disabilities, post-operative care needs, and older Australians.

About the role

We currently have positions available for qualified, reliable, and passionate Care Coordinators to join our team in Waterloo and surrounding areas. It is preferred that you are an experienced Registered Nurse with AHPRA registration or Personal Care Worker experienced Aged Care Worker or Disability Support Worker.

You will need to be friendly, and positive and demonstrate client-focused behavior, showing empathy and treating others with dignity and respect. You’ll also need to have a natural ability to build relationships that make a difference and be honest, helpful, and dependable, making you a great asset to our team.

Responsibilities will include: Working within a person-centered, enabling environment that respects the rights and needs of each client

  • Coordinating and allocating your care team and ensuring that client needs are met.
  • Observe, care and report any changes in the health and wellbeing of the client
  • Responding to emergency situations
  • Responsible for the assessment of HCP and private clients.
  • Facilitate wound dressing, complexed skilled nursing, catheter care, IV therapy, tube feeding, and palliative care
  • Coordinate with Hospitals and facilities for medical and laboratory appointments on behalf of the client's

Essential Criteria: Registered Nurse

AHPRA registered
  • Minimum Certificate III in Individual Care or equivalent
  • Excellent verbal and written communication skills
  • Willingness to learn, develop and take on new challenges
  • Current National Police Check
  • Current First Aid & CPR certificates
  • Current unrestricted driver’s license with a preferred car
  • Endorsed to administer medication (desirable)
  • Comprehensive car insurance (desirable)
  • COVID19 Vaccination (Mandatory)
