An exciting opportunity has arisen for a full-time Registered Nurse to take on the role of Clinical Care Coordinator at Derwent Views in Tasmania.

About The Role: The Clinical Care Coordinator is responsible for coordinating residents’ person-centered clinical care in line with policies, procedures and professional standards, and in collaboration with the Care Manager. The primary of purpose of the Care Coordinator is to focus on individual resident needs, ensuring that they have a current documented Care Plan, which meets their needs, and is reviewed and delivered by the multi-disciplinary care team.

The Clinical Care Coordinator works in collaboration with the Care Manager to ensure effective coordination of resident care, including:

  • Assists with the admission of new residents to the facility, ensuring that they are assessed.
  • Assess the individual needs of residents and develop person-centered care plans in consultation with other healthcare professionals, ensuring that care is tailored to meet their specific needs.
  • Collaborate with other healthcare professionals to deliver comprehensive care to residents.
  • Communicate effectively with residents' families and health care providers, keeping them informed about their health conditions, care plans, and any significant changes or concerns.
  • Maintain accurate and up-to-date resident documentation, including care plans, behaviour charts, medication charts, observations and/or incidents.
  • Provide and assist with direct nursing care to residents, including and not limited to assisting with activities of daily living (ADLs), administering and managing medication, catheterisation, conducting observations and monitoring, and wound management.
  • Actively participates in case conferences and relevant meetings
  • Complete clinical assessments and reviews at required intervals and escalate as required, including COD status, daily wound care and fall management
  • Undertake audits as required, including the Schedule 8 drug count
  • Assist with pharmacy ordering as required
  • Check alerts, overdue tasks and progress notes to ensure they are completed effectively and to standard
  • Coordinate infection control measures are in place and ongoing risk reduction strategies are being implemented
  • Assist with the investigation of clinical incidents and their close out as required
  • Collate data and prepare information for reports as required
Requirements
  • Bachelor of Nursing
  • Current Australian Health Practitioner Regulation Agency (AHPRA) registration
  • Nursing experience within a residential aged care facility
  • Experience of working cooperatively with multi-disciplinary care teams
  • Strong clinical assessment, documentation and practice skills
  • Effective communication and interpersonal skills
  • Proficient computer and information management skills, including ability to use Microsoft and clinical software systems.
  • Experience in a similar clinical care role
  • An understanding of the aged care accreditation process
  • Working knowledge of quality improvement programs
  • Current Driver’s License.
