Daily tasks include:

  1. Assessing clients' needs and planning, developing and implementing educational, training and support programs
  2. Provide guidance, training and supervision to support workers, conducting regular team meetings to ensure participant needs are met efficiently. Facilitate communication between participants, their families, and friends to support their engagement and well-being.
  3. Interviewing clients and assessing the nature and extent of difficulties collaborating closely with the Director and support workers to develop individualized support plans. This involves engaging with participants, their families, and support networks to ensure comprehensive care and effective service delivery.
  4. Monitoring and reporting on the progress of clients maintaining accurate records and documentation related to participant care, support plans, and incident reports. Ensure compliance with regulatory standards and organizational policies.
  5. Referring clients to agencies that can provide additional help, and assess community needs and resources related to health, welfare, housing, employment, and training. Liaise with community groups, welfare agencies, government bodies, and private businesses to address community issues and promote awareness of available resources.
  6. Document and report any concerns or complaints raised by participants or their representatives to the Director immediately, ensuring prompt resolution and continuous improvement of services.
  7. Providing comprehensive support and assistance to participants in their daily living activities, including personal care, meal preparation, medication management, and hygiene routines.
  8. Overseeing the day-to-day operations of the Supported Independent Living (SIL) house, including coordinating grocery shopping, meal planning, and maintaining a clean and safe living environment.
  9. Collaborating closely with the Director and a team of support workers or carers to ensure participants needs are effectively met. This may involve providing guidance and supervision to support workers and conducting regular team meetings.
  10. Developing weekly rosters, covering planned and unplanned absences.
  11. Engaging in collaborative efforts with participants, their families, and support networks to develop individualized support plans under the guidance of the Director. Responding promptly and effectively to any emergencies or crisis situations within the SIL house, such as medical emergencies, behavioural incidents, or conflicts among participants or staff.
  12. Maintaining accurate records and documentation related to participant care, support plans, incident reports, and other administrative tasks as required by regulatory standards and organisational policies and procedures.
  13. Acting as a backup SIL Facilitator for another house in case of emergency situations.
  14. Providing active support to Participants in a manner that prioritizes their control and independence without compromising Brave Minds Social Services duty of care. Implementing programs that respect the individual cultural differences of Participants from diverse backgrounds.
  15. Engaging in ongoing training and professional development opportunities to enhance skills and knowledge in Mental Health support, behaviour management, communication techniques, and relevant legislation or regulations.
  16. Ensuring business operational needs are met and preparing reports as required by management, including compliance, other weekly/monthly reports.
Requirements:
  • Minimum qualification of Diploma in relevant field is required
  • At least 1 year of working experience working in a related field, such as Youth Work, Social Work, Human Services, Community Mental Health or similar.
  • Ability to plan, implement, deliver and evaluate plans that work towards individuals NDIS goals.
  • Understanding in mental health systems and legislation as well as the Disability Services Act.

Annual salary: $75,000 - $85,000 + superannuation

Full-time position (38 working hours/week)
